Registering for the Burgundy Farm Summer Day Camp?  In addition to the online registration you will need to submit these forms.

Mandatory forms that ALL campers must submit:

  • Virginia Child Registration Form 
    • Returning camper?  Parents should fill out the first page with updated information and sign and date the second page. Include the current camp dates (or list our entire summer for the dates so you can be covered if you add any sessions of camp, June 17 – August 9, 2024).  You DO NOT need to complete the camper identification piece again.
    • Brand new camper or you did not submit this form for Summer 2024? Parents must complete the form in its entirety and arrange for camp staff to view it in person along with an original CAMPER ID document (birth certificate, valid passport, etc).  Parents can take advantage of the Paperwork Playdates or call the camp office to schedule a time to satisfy the camper ID requirement.
  • Virginia Health Form 
    • This form is valid for 2 years from the date of the physical exam on the form (i.e. May 14 2022 – May 14 2024).
    • If you have a current VA Health Form (within two years of the campers time at camp for summer 2024) you can submit a copy of the existing form.
    • This form has 3 sections: one to be completed by the parents, one to be completed by the pediatrician (wellness visit), and a copy of the campers immunization records.
    • Burgundy CAN accept forms from other states and districts as long as they are current and complete.

Optional camper forms: 

Medication Forms: only required if your camper(s) will need ANY medication administered while at camp.

  • Medication Authorization Form – Over the counter only, 10 days or less
  • Medication Authorization Form – Prescription / Over the counter, 10 days or more
    • Please note that this form has a section that must be completed by the pediatrician.
    • This form must be completed with as much details as possible with accurate medication names (generic vs. name brand), symptoms that require administration (difficulty breathing, rash, swollen tongue, etc.), specific method of administering and dosage, and dates that the form is valid for.
    • Parents must complete a Burgundy Farm Summer Day Camp Medication Authorization form. We CANNOT accept forms from other camps/schools.
    • Medications must be in their original packaging and may NOT be expired.
    • Medications and Medication Authorization Forms should be placed in a ziplock bag with the campers name clearly labelled.

Please note: the above forms are IN ADDITION to the information you will enter during the online registration process.  To be completely ready for camp you must complete the online registration AND complete and turn in these hardcopy forms.

Submitting Camp Forms

  • In late spring Burgundy will host two Paperwork Playdate events that will allow families to come to campus and take care of their camper’s paperwork requirements as well as explore the campus and meet some of the camp staff.  Families who attend these events will want to be sure to bring EVERYTHING they need to satisfy their campers paperwork requirements (the actual hardcopy forms and valid ID’s).
  • You can also upload completed paperwork via our registration site.  There is a video at the bottom of this page that shows you how.  If you choose this option please email camp admin to let them know that you have submitted your paperwork online.
